Sliding in air will allow you to heavy attack. You don’t need to keep sliding, you just need to be sliding for the time when you actually hit the heavy attack button. Also to not switch to your main weapon, you need to actually switch to your melee, not the quick swap. This is done by holding down your weapon switch button, F by default.

A contagion zaw allows you to reclaim ranged attacks whilst only having a melee equipped. I find it preferable to using a glaive as they're pretty clunky on console. Your mileage may vary.
